Short term respite for Hospice and Dementia Clients
Overnight and Weekend Stays Available. Pain management as needed. Teaching/support for family members.
Medication Management
Biweekly or Monthly Plans, teaching and communication with PCP as needed.
Post-Op Care/"Tuck in" Services
Including review of Discharge Instructions, Med Set up, Guidance on equipment needed, basic assessment of vital signs and surgical site, emptying of any drains, teaching for the client and family. "Tuck In" for the client with extended stays available.
Wellness checks
Vital Signs, Basic assessment, Teaching , Communication with Family members and PCP as needed.
Teaching and Navigation of Health services
Chronic Disease management including but not limited to : Congestive Heart Failure, COPD, Diabetes with the Goal of reducing ER and Hospital Stays. Nutrition and overall "Good Practices" Emphasized.
